Heart and circulation symptoms
Heart-related symptoms are common in Long Covid. These symptoms can have many causes. Some relate to dysautonomia/PoTS, some to breathing pattern changes, some to inflammation, and some to other issues such as anaemia or thyroid problems.
What it can feel like
palpitations (racing, pounding, fluttering, skipped beats)
chest pain or discomfort (tightness, pressure, heaviness, burning)
breathlessness or reduced exercise tolerance
dizziness, near-fainting or fainting
symptoms that are worse when upright, in heat, after meals, or after exertion
a “wired” feeling, adrenaline surges, or feeling shaky/unwell
Symptoms can fluctuate and may come in waves. Some people notice symptoms are worse during symptom flares, after poor sleep, or after infection.
It can help to note:
Position: does it happen when standing still or upright, and improve when lying down? (this can point towards PoTS/orthostatic intolerance)
Timing: does it happen during activity, or does it worsen later (for example later the same day or the next day)? (this can link with PEM)
Context: does it occur after meals, in heat, with dehydration, or in busy environments?
Associated symptoms: breathlessness, dizziness, nausea, tremor, reflux, cough, anxiety-like sensations
Duration: how long does it last, and what helps it settle?
What can help day to day
These are general, non-medical steps that some people find helpful while waiting for assessment or alongside care.
-
If symptoms worsen after activity, plan smaller bursts of activity with rest. If you experience PEM, see our PEM section.
-
Some people find symptoms flare with heat, dehydration, rushing, or large meals. Where possible, keep routines steady and avoid known triggers.
-
If you feel dizzy or your heart is racing, stop and rest. Sitting or lying down can help.

A short symptom summary (what happens, when, what helps) can make it easier for a clinician to assess.
